What is my Next Step?
If you would like to apply for admission into Thomas Holt Villages residential aged care facility, here’s what to do next:
- Complete the enclosed government forms to ensure that the appropriate fees and charges can be applied. Please see the back pages of these forms for details on where to post them. Do not send them to Thomas Holt Villages
- You may like to have a look around the facility/ies in which you are interested. If so, contact the Admissions Officer (02) 9545 4799 to arrange an appointment for Tuesdays or Thursdays – times 11am and 2pm for a tour. The Admissions Officer will be happy to show you around the facility and answer any of your questions.
- You may need more financial information regarding income tested fees, bonds and charges. If so, please call the Admissions Officer and ask to speak to the Admissions Officer.
- Prospective residents and / or family must attend an interview with the Director of Care before being accepted for admission.

- To be eligible for residential aged care, you need to be assessed by an Aged Care Assessment Team (ACAT). Your GP can organise this for you. They will give you a form called an Aged Care Application and Approval form, otherwise known as a 3020.
- Complete the forms in this kit as shown on the Checklist on page 14, and send to:

1-25 Acacia Road North Sutherland NSW 2232 - When a vacancy arises that is suitable for you, the Admissions Officer will contact you directly. You may be asked to make a decision about placement within hours.
Once Thomas Holt Villages receives your application, you will be sent a letter confirming that you have been placed on the waiting list. Please do not send incomplete applications, as Thomas Holt Villages cannot process them.
